چکیده

Aim: To explore if gadolinium-ethoxybenzyl-diethylenetriaminepentaacetic acid (Gd-EOB-DTPA)-enhanced MRI provides more accurate information than multi-detector row computed tomography (MDCT) for staging hepatocellular carcinoma (HCC). Methods: We retrospectively investigated 112 patients with HCC who underwent MDCT and Gd-EOB-DTPAenhanced MRI within one month before treatment. Two experienced radiologists reviewed the size, number, and boundaries of the HCC lesions, portal invasion, and tumor metastasis on MDCT and MRI images in consensus. Then the performance of Gd-EOB-DTPA-enhanced MRI and MDCT on staging HCC lesions was compared through statistical analysis. The classification of the Barcelona Clinic Liver Cancer (BCLC) stage was also evaluated. Results: HCC lesions in five patients were detected on Gd-EOB-DTPA-enhanced MRI only. Of the other 107 patients, 21 (19.6%) were in BCLC stage 0, 49 (45.8%) in BCLC stage A, 14 (13.1%) in BCLC stage B, and 23 (21.5%) in BCLC stage C on MDCT. On Gd-EOB-DTPA-enhanced MRI, 12 (11.2%), 48 (44.9%), 17 (15.9%), and 30 (28.0%) patients were classified as BCLC stages 0, A, B, and C, respectively. The two methods differed significantly in staging HCC patients (χ2=16.444, P=0.006). Conclusion: This study suggests that Gd-EOB-DTPA-enhanced MRI provides more accurate information than MDCT for characterizing and staging HCC which will help to choose accurate treatment strategy.

Clinical utility of imaging for evaluation of hepatocellular carcinoma

The hemodynamics of a hepatocellular nodule is the most important imaging parameter used to characterize various hepatocellular nodules in liver cirrhosis, because sequential changes occur in the feeding vessels and hemodynamic status during hepatocarcinogenesis. Therefore, the imaging criteria for hepatocellular carcinoma (HCC) are also usually based on vascular findings, eg, early arterial up...

Contrast-enhanced computed tomography plus gadolinium-ethoxybenzyl diethylenetriamine pentaacetic acid-enhanced magnetic resonance imaging for gross classification of hepatocellular carcinoma

Accurate gross classification through imaging is critical for determination of hepatocellular carcinoma (HCC) patient prognoses and treatment strategies. The present retrospective study evaluated the utility of contrast-enhanced computed tomography (CE-CT) combined with gadolinium-ethoxybenzyl diethylenetriamine pentaacetic acid-enhanced magnetic resonance imaging (EOB-MRI) for diagnosis and cl...

Health economic assessment of Gd-EOB-DTPA MRI versus ECCM-MRI and multi-detector CT for diagnosis of hepatocellular carcinoma in China

Limited data exists in China on the comparative cost of gadolinium ethoxybenzyl diethylenetriamine magnetic resonance imaging (Gd-EOB-DTPA-MRI) with other imaging techniques. This study compared the total cost of Gd-EOB-DTPA-MRI with multidetector computed tomography (MDCT) and extracellular contrast media-enhanced MRI (ECCM-MRI) as initial imaging procedures in patients with suspected hepatoce...

Gadolinium ethoxybenzyl diethylenetriamine pentaacetic Acid-enhanced magnetic resonance imaging findings of borderline lesions at high risk for progression to hypervascular classic hepatocellular carcinoma.

OBJECTIVES The objectives of the study were to assess the imaging features of hypovascular borderline lesions containing hypervascular foci on gadolinium ethoxybenzyl diethylenetriamine pentaacetic acid (Gd-EOB-DTPA)-enhanced magnetic resonance imaging (MRI) and to evaluate the ability of Gd-EOB-DTPA-enhanced MRI to diagnose high-risk borderline lesions possibly consistent with early hepatocell...

Recent Advances in the Imaging Diagnosis of Hepatocellular Carcinoma: Value of Gadoxetic Acid-Enhanced MRI.

Magnetic resonance imaging (MRI) using gadolinium ethoxybenzyl diethylenetriamine pentaacetic acid (Gd-EOB-DPTA), or gadoxetic acid for short, is a hepatocyte-specific contrast agent which is now increasingly used for the detection and characterization of focal hepatic lesions, particularly in patients at high-risk of developing hepatocellular carcinomas (HCC). In fact, several recent guideline...
